Avalon Homecare is a private home health care provider focused on meeting the needs of our patients and their families. At Avalon, our main focus is to help people stay longer in their homes by providing the necessary care and support. By working closely with your physician and family, we are able to design and implement a customized care plan.

At Avalon, we understand that each person’s needs and circumstances are unique and that is why our programs are designed to cater to each patient at a time. We spend a lot of time understanding our patients’ living conditions and talking to caregivers in order to design a comprehensive care plan that involves all parties. Our experience has taught us that the more families are involved in the care plan of their loved one, the better the outcome.

Starting Care

You can call our 24-hr referral line to schedule an phone interview. A nurse or case manager will speak to the primary caregiver to establish the need level. Following the interview, the nurse will make a visit if necessary for further assessment. Services can begin within 24-48 hours from initial contact.
